About Mastectomy

In simple language, Mastectomy refers to the surgical elimination of the entire breast. This procedure can be applied on both men as well as women. In most of the case, this surgical procedure is done to treat breast cancer. There are different types of mastectomy including:

  1. Total (simple) mastectomy - Removal of breast tissue and nipple
  2. Modified radical mastectomy - Removal of the breast, most of the lymph nodes under the arm, and often the lining over the chest muscles
  3. Lumpectomy - Surgery to remove the tumour and a small amount of normal tissue around it

Mastectomy is an option for those individuals who have:

  • Ductal carcinoma in situ (DCIS)
  • Early breast cancer
  • Locally advanced breast cancer
  • Inflammatory breast cancer
  • Paget disease of the breast (Paget disease of the nipple)

The More Common Mastectomy Procedures Nowadays Are:

Partial Mastectomy

In a partial mastectomy, the surgeon removes a larger portion of the breast as compared to in the lumpectomy — perhaps a whole segment or quadrant of tissue — in order to remove the cancer. Occasionally, the surgeon will remove some of the lining over the chest muscles as well.

Skin-Sparing Mastectomy

This procedure requires removal of the breast, nipple, areola, and sentinel lymph node (or nodes) but not the breast skin. Many women who intend to have breast reconstruction will opt for this procedure.

Simple Mastectomy (also known as total mastectomy)

This surgery requires removal of the breast, nipple, areola, and sentinel lymph node or nodes. It leaves the chest wall and more distant lymph nodes intact.

Modified Radical Mastectomy

This procedure requires removal of the entire breast, nipple, areola, and axillary lymph nodes but often leaves the chest wall untouched.

Breast Reconstruction

A large number of women choose to have breast reconstruction to help restore the look of the breast that was removed. Reconstruction may be performed at the same time as the mastectomy (immediate) or later (delayed). It is necessary to discuss your reconstruction options with your plastic surgeon before your breast surgery. Some women choose not to have reconstructive surgery or to do it later.

When no reconstruction is scheduled, the surgeon will leave the area as flat as possible so that breast prosthesis can be easily fixed to the chest.

Breast Prosthesis

If one doesn’t wish to undergo reconstruction, he or she can get breast prosthesis. This is a breast prototype made of silicone gel, foam or other materials that can be fixed to your chest. The form is either placed directly on top of your skin or in the pocket of a special bra. Your health care provider can discuss breast prosthesis options with you and help you choose the type that best fits your lifestyle. Your prosthesis can be properly fitted several weeks after your mastectomy surgery.
