Liver Transplant Procedures and Liver Disease Symptoms

In today’s fast-paced world, we humans claim to have achieved superior standards of living from using high-end technology to living in an ultra-luxurious environment. But on the other side, we see that our health has been suffering a lot due to the prevalence of various diseases those were once not there. For example, liver diseases are on the rise.

Though there could be several reasons behind liver damage, the most commonly known ones are alcohol consumption, contaminated food intake, smoking, contaminated water, obesity, iron overload, hepatitis A, B, C, D and E and others.

The most common visible signs of liver diseases include:

  • weakness and fatigue,
  • weight loss,
  • nausea,
  • vomiting, and
  • yellow discoloration of the skin (jaundice)

Liver disease refers to any kind of disturbance in the functioning of the liver and is mainly treating by using medicines that are found to be quite effective in relieving the symptoms of a disease. But sometimes the situation gets so severe and liver transplant remains the only possible solution to save the person. There are various systems doctors rely on to determine the need for liver transplant.

It is extremely necessary for liver transplant candidates and their families to comprehend the basic process associated with liver transplants, to gain some of the challenges and complexities that face liver transplant recipients (people who receive livers), and to determine symptoms that should alert recipients to get medical assistance.

Some basics are given below:

  • The liver donor refers to the individual who provides or donates, all or portion of his or her liver to the needy patient who needs it. Donors are generally people who have deceased and want to donate their organs. Some people, however, donate part of their liver to another individual (often a relative) while living.

  • Orthotropic liver transplantation indicates a procedure in which a disastrous liver is eliminated from the patient's body and a healthy donor liver is transplanted into the same place. This procedure is the most widely used method used for liver transplantation in India.

  • With a living donor transplant, a healthy individual donates part of his or her liver to the receiver. This procedure has been progressively successful and depicts promise as an option to avert long waiting times due to a shortage of liver donors. It is also an option for children, partially because child-sized livers are in low supply. Other methods of transplantation are used for people who have possibly reversible liver damage or as short-term measures for those who are expecting liver transplants. These other methods are not discussed in detail in this article.

Over the past few years, India has become a hot favorite destination for people looking for liver transplantation in a cost-effective manner. Backed by highly experienced doctors and world-class healthcare technology, India provides superb healthcare options to people from all walks of life. From easy accommodation to high success rate, there are many reasons why liver transplantation in India is a feasible and beneficial choice from all sides. Total cost of liver transplant in India is affordable by people which is the main reason of popularity.

Doctors in India refer to various methods to determine whether the person needs a liver transplant or not. Some of these techniques include:

  • Status 1 (acute severe disease) refers to a patient with the only fresh evolution of liver disease who is in the intensive care unit of the healthcare center with a life expectancy without a liver transplant of fewer than a week or someone who got a liver transplant and the donor organ never functioned easily.

  • MELD scoring: This system is based on the chance or probability of death within 3 months if the patient doesn’t get a liver transplant. The MELD score is measured based only on laboratory data in a bit to be as neutral as possible. The laboratory values used are a patient's sodium creatinine, bilirubin, and international normalized ratio, or INR (a measure of blood-clotting time). A patient's score can vary from 6 to 40. In case of a liver becoming available to 2 patients with the same MELD score and blood type, time on the waiting list becomes the deciding factor.

  • PELD scoring: This system is based on the risk or probability of death within 3 months if the patient doesn’t get a liver transplant. The PELD score is measured based on laboratory data and growth parameters. The laboratory readings used are a patient's albumin, bilirubin, and INR (a measure of blood-clotting capability). These values are employed in conjunction with the patient's rate of growth failure to ascertain a score that can range from 6 to 40. As with the adult system, if a liver were to become ready to two same sized patients with the same PELD score and blood type, the child who has been on the waiting list the lengthiest will receive the liver.

  • Given this system, livers are first given locally to status 1 patients, then basis the patients with the maximum MELD or PELD scores. Patients on the local list with a MELD score above a specific level are offered the liver first, then it is assigned to regional and national listed patients. Once that list is bushed, the liver is given to other patients on the local regional, and national level. There are continuing discussions to adjust the liver allocation process to ensure that the sickest patients get them first, irrespective of their location.

  • Status 7 (inactive) is referred as patients who are measured to be temporarily inappropriate for transplantation.
